![]() The GRID Dental Corporation is a separate company that provides dental network services on behalf of Horizon Blue Cross Blue Shield of New Jersey, an independent licensee of the Blue Cross Blue Shield Association. Claims are paid by the Blue Plan where the member’s group is located. Primary claims, including claims using a legacy provider ID (TIN+suffix), behavioral health claims and claims requiring a medical record, can be submitted from the Horizon BCBSNJ page after logging into. Use Payer ID 22099 if you use a vendor or clearing house. Send claims to the address on the member’s ID card. All claims should be submitted electronically. Members with access to a GRID Network will have GRID or GRID+ on their dental ID card, along with a phone number to call for benefit or eligibility questions.
